Decision summary

Summary of allegations

That you, a Qualified Social Worker, employed by Denbighshire County Council ("the Council") as Team Manager – Intake and Intervention, Education and Children's Services:

1. Between 2 May 2017 and 20 March 2018, you failed to carry out statutory Looked After Child (LAC) visits in relation to Child A every six weeks.

2. At a LAC review meeting in relation to Child A on 14 August 2017, you stated, and/or allowed the LAC Review Officer to conclude, that you had continued to undertake statutory LAC visits every six weeks, despite the fact that you had not done so between 2 May 2017 and 3 August 2017.

3. On 17 April 2018 you entered onto the Council's Paris system statutory LAC visit records in respect of Child A for the following dates, despite the fact that visits had not taken place on those dates:

(a) 6 June 2017;

(b) 6 July 2017;

(c) 6 September 2017;

(d) 11 October 2017;

(e) 15 November 2017;

(f) 12 December 2017;

(g) 22 January 2018;

(h) 20 March 2018.

4. On 17 April 2018 you completed and signed a Case Transfer Audit in relation to Child A in which you stated that all visits were up to date when that was not the case.

5. On 19 April 2018, you retrospectively created entries in your electronic calendar which referred to visits to Child A on the following dates:

(a) 6 June 2017;

(b) 6 July 2017;

(c) 6 September 2017;

(d) 11 October 2017;

(e) 15 November 2017;

(f) 12 December 2017;

(g) 20 March 2018.

6. On an unknown date, you entered onto the Council's Paris system a statutory LAC visit records in respect of Child B which you stated had taken place on 18 August 2017, but which did not take place on that date.

7. Your conduct in 2 and/or 3 and/or 4 and/or 5 and/or 6 above:

(a) was inaccurate and/or misleading,

(b) lacked integrity, and/or

(b) was dishonest.

And your fitness to practise is impaired by reason your serious misconduct.
