1. You must inform the following individuals/organisations that you have conditions imposed on your registration under the Social Care Wales Fitness to Practise procedures and disclose the conditions to them:
i) Any organisation or person employing, contracting with, or using you to undertake social work or social care work;
ii) Any social care agency you are registered with or apply to be registered with (at the time of application);
iii) Any prospective social care employer (at the time of application)
iv) Any educational establishment at which you are undertaking a course connected with social work or social care, or any such establishment to which you apply to take such a course (at the time of application).
2. You must inform Social Care Wales within 5 working days of accepting any social work/social care appointment (whether paid or unpaid) which requires registration with Social Care Wales and provide Social Care Wales with contact details of the new employer, the job title of the role you have accepted, and the address of your new place of work.
3. Within 5 working days of accepting any social care/work appointment, you must also provide a signed (by your manager) written confirmation from your new employer that they are aware of the condition/s imposed and that they are willing to support you to comply with the conditions
4. You must inform Social Care Wales of any professional investigation started against you and / or any professional disciplinary proceedings taken against you within 2 days of you receiving notice of them
5. You must, within three months, successfully undertake a training course (or combination of courses) which include the following topics: professional boundaries, safeguarding and risk management and provide written evidence of attending and passing the required training. Social Care Wales must formally approve any training or learning before you begin any course or learning, and all training should be delivered in a Welsh context.
6. When working as a social worker or social care worker / manager, you must remain under the supervision of a workplace line manager, mentor or supervisor nominated by your employer for 18 months. The supervision meetings should include;
i) your adherence to, and maintenance of, professional boundaries with users of care and support;
ii) your understanding of, and effective practice in relation to, safeguarding of users of care and support, and particularly around risk assessment.
If the manager/mentor/supervisor changes you should confirm to the Fitness to Practise Officer in writing that they have been made aware of the condition/s.
If the manager/mentor/supervisor changes you should send to Fitness to Practise Officer a written confirmation (on headed paper) signed by your new manager/mentor/supervisor that states clearly they are aware of the conditions to be adhered to and that they support the compliance of the conditions.
7. You must meet with your line manager, mentor or supervisor at least every month to discuss the standard of your performance. If the manager/mentor/supervisor changes you should submit a written confirmation (on headed paper) signed by your new manager/mentor/supervisor that states clearly they are aware of the conditions to be adhered to and that they support the compliance of the conditions.
8. You must ask your line manager, mentor or supervisor to provide a written report setting out the standard of your performance to Social Care Wales every three months and at least 28 days before any review hearing. If the manager/mentor/supervisor changes you should submit a written confirmation (on headed paper) signed by your new manager/mentor/supervisor that states clearly they are aware of the conditions to be adhered to and that they support the compliance of the conditions.
9. You must consent to Social Care Wales to exchange, as necessary, information about the standard of your performance with your line manager, mentor or supervisor or any other person who is or will be involved in your training and supervision with any employer, prospective employer, and with any educational establishment.
